security: remove leaked production credentials from docs

Replace live Steadfast/Pathao/RedX/Paperfly/Google-Maps keys, Pathao login
and Paperfly basic-auth in README.md + MASTER_OPERATIONS_MANUAL.md with
<PLACEHOLDER> tokens. These belong in env/secrets, never in committed docs.
